Professional Development Documentation
2009 ASTA National Conference
March 19-21, 2009
Atlanta, Georgia

As a service to its members, ASTA will create and maintain a permanent and ongoing record of your professional development hours once you have enrolled in the program. There is a one-time enrollment fee of $25 for this program. In order to document your attendance hours, you must record the name of each session you attend and obtain the signature of each session’s clinician or presider on the two sided from which you will find in your on-site conference registration packet. Forms are due back to the national office by April 30, 2009, at which time your hours will be calculated and recorded. You will then receive a certificate, if purchased, showing your completed hours of professional development for the 2009 National Conference. Certificates are $10 and will be mailed approximately two weeks after receipt of your completed form.

Enrollment in this program also affords you access to your entire history of documented professional development hours at ASTA events when you require a record of your professional development accomplishments, as long as you maintain your membership in ASTA.
